As she stepped onto what appeared to be a covered manhole, the cover suddenly flipped over, causing her to plunge into the drainage system below.

Rosa had just gotten off a motorcycle taxi when the incident occurred.

The rider immediately rushed to help after seeing her disappear into the opening, while a passerby also ran to the scene.

Emergency crews were called and eventually managed to pull her to safety.

"I thought I was going to die, because the water was hitting my chest and the hole was very deep," Rosa told Brazilian media.

According to reports, Rosa suffered multiple injuries when the heavy cover struck her body as she fell.

Local media claim the manhole cover may have been incorrectly repositioned after allegedly being moved by metal thieves overnight.

Security camera footage from around 2 a.m. reportedly shows two men removing the cover before placing it back improperly.

Similar Tragedy Occurred Weeks Earlier in New York

The accident comes just weeks after a fatal manhole incident in New York City.

In May, Donike Gocaj, 56, died after falling into an open utility manhole in Midtown Manhattan.

The Westchester grandmother reportedly plunged around 10 feet. This was as she stepped out of her parked Mercedes-Benz SUV near East 52nd Street and Fifth Avenue.

The incident occurred at approximately 11:20 p.m.

Officials later confirmed the opening belonged to Con Edison.

"We are deeply saddened to confirm that a member of the public has died after falling into an open manhole," a spokesperson for Con Edison told the New York Post.

"We are actively investigating how this occurred. Our thoughts are with the individual's family, and safety remains our top priority."

Emergency responders found Gocaj unconscious and unresponsive inside the utility hole.

She was transported to NewYork-Presbyterian/Weill Cornell Medical Center, where she was later pronounced dead.
